  • What is Family-Managed Home Care?

    Family-Managed Home Care gives you more flexibility and choice over how you spend your funding. You can be in charge and take control over your services by finding, hiring, and paying your own service providers. You have the freedom to choose your provider, direct how they care for you (or your loved one) and set a schedule that works best for you.

  • How can DirectFunded help me?

    DirectFunded can help you and your family manage your funding by taking care of all the required bookkeeping, payments, remittances, and reports you need to submit. We will support you through the required financial management, which can often be cumbersome and time-consuming, to allow you to focus on finding the best supports and to ensure that you get the most out of your Family-Managed Home Care.

  • How much does DirectFunded cost me?

    It costs you NOTHING. The Ministry of Health and Long-Term Care (MOHLTC) provides funding for your bookkeeping needs. This funding is in ADDITION to the funds you receive to purchase hours of service.

Do you currently receive home care from the Local Health Integrated Network (LHIN)?


And Belong to one of the
four following groups?

Clients in extraordinary circumstances

Eligible home-schooled children

Adults with acquired brain injuries (ABI)

Children with complex medical needs

If so, you are likely to be eligible for Family-Managed Home Care. Click on one of the above four groups to determine your eligibility.
