Am I eligible for Self-Managed Attendant Care?

Do you have a permanent physical disability, and are at least 16 years of age?


&

Require assistance with at least two activities of daily living listed in 1 to 8 below?

(at least one of the activities must be listed in 1 to 4)

Dressing or undressing

Catheterization, emptying and changing a leg bag, using the toilet, urination or bowel routines

Washing, bathing, showering, shaving or personal grooming​

4

Breathing, or caring for a tracheostomy or respiratory equipment

5

Turning in bed, lifting, positioning or transferring

6

Essential communication

7

Meal preparation, dish washing, laundry or other housekeeping tasks

Eating

If so, you are likely to be eligible for Self-Managed Attendant Care.
